John T. Blake, MBA, CPA

INDEPENDENT DIRECTOR NOMINEE

Mr. Blake has agreed to serve on our board of directors as an independent director following the completion of this offering. He brings over 20 years of leadership experience in accounting and corporate finance within the life sciences industry, guiding companies through pre-clinical, clinical, and commercial stages. His expertise includes strategic and operational responsibilities, public and private financings, mergers and acquisitions, and public-company compliance. He currently serves as Senior Vice President, Head of Finance for Pulmovant, a biotechnology company focused on respiratory disease therapies. Previously he was Chief Financial Officer of Provisio Medical (April 2020 - December 2024), where he led the company through FDA clearance of its endovascular measurement device. Prior to Provisio, he was Chief Financial Officer of Second Sight Medical Products (Nasdaq: EYES) from March 2018 - September 2020.  Prior to that, he was Senior Vice President, Finance at aTyr Pharma(Nasdaq: LIFE), playing a key role in its $86 million initial public offering. Earlier in his career, he held global finance leadership roles at Volcano Corporation (Nasdaq: VOLC), which was acquired by Philips for $1.2 billion. Mr. Blake is a Certified Public Accountant and holds an MBA from the University of Southern California
